EDUCATIONAL FIELD TRIP PERMISSION SLIP

 

Dear Parent(s) or Guardian(s):  

We have been planning an exciting educational field trip for your child’s class. 

 

We will be having a field trip to Toronto at the Royal Ontario Museum (ROM) on Wednesday, February 22nd, 2023.

 

Alfajrul Bassem Academy’s policy requires each child to have advance written permission to go on a field trip. Students who do not have prior written permission will not be able to participate in the field trip.

 

Students are asked to pack a lunch, water bottle and a snack.

 

NOTE TO PARENTS AND STUDENTS:

 

Please complete the lower section of this form with the attached waiver and return it to school immediately.

Cost: 22 $ 

Drop off and pick up time will be the same as a normal school day.

 IMPORTANT: Students must arrive ON TIME by 8 am. 

 

Your child(ren) will need: coats, gloves, hats, snacks, water bottle, light back-packs, and proper running shoes. 

 

Mode of transportation: students will be transported using school transportation to GO station then will be taking the Go train to Toronto. and will be using the subway for lunch.
